Do offshore casinos actually pay out?

Short answer
The good ones do, reliably and quickly. The problem is that from the outside a site that pays and a site that does not can look identical, and there is no regulator to force a payout if one refuses. That is why a tested payout record matters more than any licence badge or bonus.

This is the question that matters most, and it deserves an honest answer rather than a reassuring one. Many offshore casinos pay out reliably. Some do not. And the hard part is that you usually cannot tell which is which from the marketing, because the sites that will not pay look exactly like the ones that will.
Why there is no simple guarantee
At a licensed casino in your own country, a withheld withdrawal can be escalated to a regulator that will investigate and, if needed, order the operator to pay. Offshore, that backstop is weak or absent. On an Anjouan or Costa Rica licence there is effectively no authority that will take up a player's dispute, so if an operator decides not to pay, the practical recourse is close to zero.
